Registration & LicensingAll HealthActCHQ users are asked to register and be approved for survey licensure. Complete information is required during registration to process a request for pricing and licensure.
HealthActCHQ has established a standard, non-modifiable licensing agreement used internationally. All requests for project information to secure licensure are treated in complete confidence. U.S. and International copyright laws protect all HealthActCHQ surveys. This includes the survey forms themselves, short-forms, scoring algorithms, translations, and derivative works, etc. All rights are reserved.
With prior approval, HACHQ Survey Licenses permit the Surveys to be accessible electronically on servers owned and controlled by a Licensee. Access is restricted to patients and enrollees recruited and known to the Licensee and requires password-protected access to complete the Surveys. All Internet access to the Surveys shall cease upon the expiration or termination of the Licensing Agreement. Determination of the licensing fee is based on the complete information provided during the registration process - including proposed use, funding source, sample size, number of survey administrations, the number of study sites, start and end dates of the project, and survey languages to be licensed. |
Single Site, Unfunded Academic ResearchUnfunded, academic users surveying less than 200 patients and/or families are required to register and pay a nominal fee to be granted a license authorizing use of HACHQ surveys and to obtain a User's Manual and scoring documentation for the surveys. Funded ResearchPharmaceutical company clinical trials, patient registries, multi-site funded academic research, hospitals, foundations, school systems, clinics, managed care organizations, contract research organizations, software developers, disease management companies, and others are required to have a paid, standardized licensing agreement in place for the authorized use of HealthActCHQ surveys for each project. Registration provides potential licensees with the terms and fees associated with licensure. You are not obligated to purchase by registering with HACHQ. Prices quoted during this online registration process will be valid for a period of one year to enable grant submissions. Licensing requests beyond this time period must be registered again and may be subject to price changes. Licensing Agreement terms are available for review with a fully completed registration. A Licensee is required to agree to the terms of licensure prior to their final purchase of a limited-use license for HACHQ survey use. |
Each distinct research project requires its own license granting limited use of the survey instruments. This includes academic, pharmaceutical, clinical, and government Licensees. Separate licensure is required for each phase of a clinical trial, for extension studies, and for marketing purposes. All users are required to have a current license acquired from HealthActCHQ authorizing use of the survey in any application and setting. Manuals, scoring rules and CD's, surveys, and licenses are not transferable.
HealthActCHQ surveys may not be abridged, modified, amended, altered, adapted, or translated without prior written approval from HealthActCHQ Inc. It is unlawful to distribute copies of the surveys, including translations, scoring documents, manuals, etc. to third parties by sale, rental, lease, lending, or any other means. Each licensee agrees to score the HACHQ surveys according to the specific rules as provided by HealthActCHQ with each paid license and as published in the User's Manual provided with each survey license. Any negotiations for use, paid or unpaid licensing, and all business arrangements regarding the surveys are the exclusive rights of HealthActCHQ. All surveys must display the specific copyright information provided with the surveys and each license.
