Registration & Licensing
   

All HealthActCHQ users are asked to register and be approved for use. Each licensee agrees to score the CHQ and other surveys according to the specific rules as provided by HealthActCHQ and published in the Users Manual for each survey.

Each distinct project requires a license granting use of the survey instruments. All users are required to have a current license acquired from HealthActCHQ authorizing use of the survey in any application/setting. Manuals, scoring, surveys, and licenses are not transferable.

HealthActCHQ has established a standard, non-modifiable agreement used internationally for all licenses. All requests for licensure are treated in complete confidence. U.S. and International copyright laws protect all HealthActCHQ surveys, including but not limited to the AIM-A, AIM-C, CHQ, FIPRE, ITQOL, NICU FITS, PAVE, PEMQOL, and PRO-SF1. This includes the forms themselves, short-forms, scoring algorithms, translations, derivative works, etc. All rights are reserved.

HealthActCHQ surveys may not be abridged, modified, amended altered, adapted, or translated without prior written approval from HealthActCHQ Inc. It is unlawful to distribute copies of the surveys, including translations, scoring documents, manuals, etc. to third parties by sale, rental, lease, lending, or any other means. Any negotiations for use, paid or unpaid licensing, and all business arrangements regarding the surveys are the exclusive rights of HealthActCHQ. All surveys must display the copyright information provided with the surveys and each license.


Single Site,
Unfunded Academic Research

Unfunded, academic users are required
to register and pay a nominal fee to purchase surveys and obtain a User’s Manual and scoring documentation for HACHQ surveys.

Funded Research

Pharmaceutical company clinical trials, patient registries, multi-site funded academic research, hospitals, foundations, school systems, clinics, managed care organizations, contract research organizations, software developers, disease management companies, and others are required to have a paid, standard licensing agreement in place for the use of HealthActCHQ surveys for each project or use. Determination of the licensing fee is based on information provided during the registration process, including use, funding source, sample size, number of survey administrations, and the number of study sites and languages to be used.

Registration provides potential licensees with the terms and fees associated with licensure. You are not obligated to purchase by registering. Prices quoted during this online registration process will be valid for a period of one year. Licensing requests beyond this time period must be registered again and may be subject to price changes.
